Dobutamine and the coronary vasomotion paradox.

چکیده
Dobutamine, a structural relative of dopamine, acts directly on alphaand beta-adrenergic receptors without the release of norepinephrine or activation of dopaminergic receptors. For clinical use, dobutamine is a racemic mixture of a levo-isomer acting as a potent alpha-1 agonist and a dextro-isomer counterbalancing this effect as an alpha-1 antagonist. Both isomers are complete beta-receptor agonists with the dextro-isomer 10-fold more potent than the levo-isomer (1). Intravenous dobutamine is widely used to determine the significance of coronary atherosclerotic disease (2) and to support the myocardium during heart failure (3). Depending on dose, dobutamine has been demonstrated to evoke myocardial ischemia and identify myocardial hibernation and viability (4,5).

Coronary Artery Spasm During Dobutamine Stress Echocardiography: A Case Report
Dobutamine stress echocardiography (DSE) has been widely used as a diagnostic and prognostic modality in the management of stress. DSE is associated with limited complications and adverse effects on the health of patients. In this case report, we described a 42-year-old female patient with dobutamine-induced coronary artery spasm with history of exertional dyspnea, which had deteriorated recent...
متن کاملThe influence of left ventricular geometry on coronary vasomotion in patients with essential hypertension.
The objective of this study was to assess the influence of left ventricular (LV) geometric pattern on coronary vasomotion in patients with essential hypertension. We studied 34 hypertensive patients, who had never been treated, with angiographically normal coronary arteries. Patients were classified into four LV geometric patterns by echocardiography: normal, concentric remodeling, eccentric hy...
متن کاملAngiotensin type 1 receptor antagonism reverses abnormal coronary vasomotion in atherosclerosis.
OBJECTIVES This study was performed to determine whether angiotensin type 1 (AT1) receptor inhibition improves abnormal coronary vasomotion and endothelial dysfunction in patients with atherosclerosis or its risk factors. BACKGROUND Endothelial dysfunction, an early feature of atherosclerosis, contributes to abnormal vasomotion during stress. Angiotensin II may contribute to endothelial dysfu...
متن کاملReversal of abnormal coronary vasomotion by calcium antagonists in patients with hypercholesterolemia.
BACKGROUND It has been shown that exercise-induced coronary vasodilation of angiographically normal coronary vessels is reduced in hypercholesterolemic patients. The purpose of this study was to evaluate the effect of calcium channel blockers on coronary vasomotion of angiographically smooth coronary arteries in hypercholesterolemic patients. METHODS AND RESULTS A total of 57 patients were in...
متن کاملVasomotion and nitric oxide bioactivity in diseased coronary arteries.
Atheromatous coronary stenoses are no longer considered to be passive structures, instead having the capacity for dynamic, often transient, change which may take the form of constriction or dilatation in response to either endogenous or external stimulation.
